Jazzy Eyes Vision Plan

In response to the ongoing pandemic we have seen many patients fearful that they will be losing their health insurance and vision plan.

We are happy to announce that we have come up with a vision plan that for patients who will no longer be able to have one through their medical insurance plans.

This is a vision plan meant to help with the cost of your annual eye examination, glasses and contact lenses. This is not a medical eye plan.

All medical conditions of the eye will need to go through your medical insurance plan.

Platinum Plan

Plan Cost $189

One comprehensive eye examination per year including an eye pressure check, dilation, retinal photography and retinal scan with OCT with one follow up visit included. 25% off on frames and lenses. 15% off customary contact lens fitting fees

Gold Plan

Plan Cost $159

One comprehensive eye examination per year including an eye pressure check and dilation with one follow up visit included. 20% off on frames and lenses. 10% off customary contact lens fitting fees.

Terms and Conditions

Annual fee and must be paid prior to the eye exam. Up to 2 pairs of glasses per year. Some exceptions apply. Cannot be combined with other insurance plans.
